What Is CoQ10?

Coenzyme Q10 (CoQ10) is a fat-soluble compound that every cell in your body produces. It has two jobs. First, it shuttles electrons inside your mitochondria — the power plants of your cells — to generate ATP, the energy currency your body runs on. Without CoQ10, your mitochondria sputter and your cells run out of gas. Second, it acts as a powerful antioxidant, protecting your mitochondrial membranes and LDL cholesterol from oxidative damage.

How It Works

The Longevity Connection

Cardiovascular Disease and Mortality

The strongest evidence for CoQ10 and longevity comes from cardiovascular trials — because heart disease is still what kills most of us. The Q-SYMBIO trial (Mortensen et al., 2014) randomized 420 patients with moderate-to-severe heart failure to 300 mg/day CoQ10 or placebo for 2 years. Cardiovascular mortality was 43% lower in the CoQ10 group (p=0.005), and all-cause mortality was 42% lower (p=0.01). The number needed to treat to save one life was just 12.

The KiSel-10 study (Alehagen et al., 2013) took a different approach: 443 healthy elderly Swedes (mean age 78) received 200 mg/day CoQ10 + 200 mcg selenium or placebo for 4 years, with follow-up extending to 5.2 years. Cardiovascular mortality dropped 53% in the treatment group. Even more striking: the benefit persisted at the 10-year follow-up — 10 years after the supplements were stopped — suggesting CoQ10 may have triggered lasting physiological changes.

Cognitive Aging

A 2026 randomized controlled trial (PMID 42510537) gave 111 adults aged 60+ either 200 mg/day ubiquinol or placebo for 90 days. The ubiquinol group showed significant improvements in cognitive function scores, reductions in blood pressure, and improvements in arterial stiffness. Blood levels of CoQ10 increased 4.5-fold, confirming absorption. This is one of the first trials to specifically test ubiquinol (not ubiquinone) in an elderly population, and the results suggest the reduced form may be more effective for brain aging.

Earlier studies provide context but are less conclusive. A 2019 meta-analysis of 7 RCTs found CoQ10 improved cognitive function in older adults with mild cognitive impairment, but the effect size was modest and study quality was mixed.

Statin-Induced CoQ10 Depletion

Statins block HMG-CoA reductase — the same enzyme that produces cholesterol also produces CoQ10. Multiple studies confirm that statin users have 16–54% lower serum CoQ10 levels. This depletion is one proposed mechanism for statin-associated muscle symptoms (SAMS), which affect 5–29% of statin users. A 2018 meta-analysis of 12 RCTs found that CoQ10 supplementation significantly reduced statin-induced muscle pain (SMD -0.53, p=0.001). Mitochondrial Function and General Aging

This is where the evidence gets thinner. While the logic is compelling — declining CoQ10 → mitochondrial dysfunction → aging — we lack a direct human RCT showing CoQ10 extends healthspan or lifespan in healthy adults. Animal studies show mixed results: CoQ10 extends lifespan in some accelerated-aging mouse models but not in normal wild-type mice. The KiSel-10 long-term follow-up provides the closest thing to human longevity evidence, but it's still cardiovascular-specific.

Ubiquinone vs. Ubiquinol: Which Form Is Better?

Ubiquinone is the oxidized form — it's what your body produces and what most older studies used. It's cheaper and well-tested. But it has notoriously poor absorption: standard powder capsules achieve only 2–3% bioavailability. Softgel formulations with oil improve this to ~6%.

Ubiquinol is the reduced, electron-rich form. Because it's already in the active antioxidant state, it doesn't require conversion in the body.
